What Is Care Management?
Care management is a health care program provided to patients at no additional cost. It covers a set of activities intended to improve your care and reduce the need for medical services. We accomplish this by working closely with your doctor and care team to make sure you have the tools necessary to manage your health.
At Rush Health, you will have the opportunity to work with our care management team which consists of registered nurses, clinical social workers, registered dieticians and patient care navigators. While we do offer a lot of help to our patients, below are just a few examples of how joining our Care Management program can benefit you:
- Care coordination between your primary doctor and specialist(s)
- Patient education and coaching to help you understand how to better manage your health
- Access to medication tips and resources that will help you take your medication as prescribed
- Resources to help you avoid barriers that may prevent you from reaching your health care goals
- Nutrition counseling for patients interested in changing their eating habits
- Interactive Diabetes Education program for patients diagnosed with Type 1 or Type 2 diabetes
